FOR THE DESIGN AND PERMITTING

The Town Administrator for the Town of Otis, on behalf of the Department of Public Works hereby requests the submittal of qualifications and proposals from qualified professional engineering firms to evaluate and perform design work for the Reservoir Road Culvert Replacement Project. Proposal price to include all work required to execute an approved Chapter 85 design and to advance this project to bid opening. Reservoir Road is a public way, 2.99 miles in length that provides access to residential properties, camps, Tolland State Forest and Otis Reservoir. The feature in the proposed project is outfall from Otis Reservoir into Farmington River. Current conditions include culvert misalignment, plunge pools on inlet and outlet sides, headwall cracking, and headwall rocks on outfall sides are undermining. Indications from a prior event demonstrate the culverts are not able to handle emergency flow. The project shall include removing the two (2) existing steel culverts and replacing with a single span open-bottomed structure, establishing a natural stream channel; improving the integrity of the berm along the river; installing new guardrails; and, restoring the roadway surface upon completion. All work is to be in accordance with the current edition of Mass DOT’s Construction Standards, Standard Specifications for Highways and Bridges, Massachusetts River and Stream Crossing Standards, and Supplemental Specifications, unless otherwise noted. Submission to Mass DOT in accordance with M.G.L. Ch. 85 Sec. 35 and 25% design would be required to the Mass DOT District Office within 6 months.
Consultants intending to submit proposals on this project must be prequalified in intermediate bridge design by the Mass DOT Highway Prequalification Office, Room 6260, 10 Park Plaza, Boston, MA 02116.
The contract and its award process are not subject to the Uniform Procurement Act pursuant to the M.G.L. c. 30B or the Designer Selection Law, Chapter 7C. Issuance of this Request for Proposal does not signify the Town’s Submission of this service to the provisions of the Act, and the contract and award will be made at the Town’s discretion, by Qualified Based Selection.
Proposals are due September 08, 2020 at 2:00 pm to the Town Administrator, 1 North Main Road, P.O. Box 237, Otis, MA 01253. Proposals will be opened and read immediately following closing time and will not be public. Contract will be awarded within 30 days. All proposals must be received in a sealed envelope properly marked “Reservoir Road Culvert Project” and be received prior to due date and time. The Town of Otis reserves the right to reject any and all proposals when it deems it to be in the best interest of the Town. INVITATION TO BID
The Town of Otis will receive bids for Mass DOT approved applications for the Full Depth Reclamation & Paving of sections of West Center & Stebbins Rd. until 1:00pm on Thursday September 10th. Specifications are to be requested in writing from TownAdmin@townofotisma.com. Bids are to be submitted to the Board of Selectmen’s Office, Otis Town Hall, 1 North Main Road, P.O. Box 237, Otis, MA 01253 on or before the above date and time. The bids will be publicly opened and read aloud on Thursday September 10th at 1:00 pm. Bids are to be submitted in sealed envelopes plainly marked: INVITATION FOR BID – WEST CENTER/STEBBINS ROAD PAVING. Bid prices shall include all labor, equipment and material for a complete in place product at West Center & Stebbins Road. Bid prices are to be based on FDR, application of HMA Binder & Top mix along with Tack coat in between courses. The project consists of two sections approximately 5230’ x 22’ and 1600’ x 19’ of FDR of the existing blacktop surface and the application of a HMA Binder course of two inches and HMA top course of one and a half inches approximately 2700 tons, along with tack coat at .05 gallons a square yard in between layers. Wedge joint to be installed on each layer of HMA as well as tacked between courses. The base price for liquid asphalt shall be $485.00 per ton and the base price for diesel shall be $1.56 per gallon. Price adjustment clause in effect for hot mix asphalt mixtures. Bids are to include base price of liquid asphalt and diesel and must be OSHA 10 certified. Project to be completed per current Mass DOT specifications. Project is to be bid as one package. Prevailing wage applies as per M.G.L. Chapter 149, Section 26 to 27F and a wage schedule is included in the specifications. A 100% performance bond is required of successful bidder, 5% bond required with bid. The Town of Otis is accepting bids by September 22, 2020 at 2:00 pm for the following services in accordance with Chapter 149, Massachusetts General Law.
"BOILER AND BURNER MAINTENANCE SERVICES PROVIDER"
The contract being offered is for a one year period from October 15, 2020 through October 14, 2021 with two (2) additional one (1) year options at the sole discretion of the Town. Services to include preventative maintenance and emergency service on 13 boilers/burners located at 9 individual sites within the Town of Otis. All contracts must be strictly awarded in accordance with the requirements of this Invitation for Bid. If it becomes necessary to revise any part of this Bid or otherwise provide additional information, an addendum will be issued to all prospective bidders who received copies of the original request. Yearly services are not expected to exceed $5,000.
Chapter 149 applies to the services to be provided. Full compliance with Federal, State and Municipal Wage Laws is required of all work done for the Town of Otis. Minimum Wage Rates as determined by the Commissioner of Department of Labor and Industries under the provision of the Massachusetts General Laws, Chapter 149, Section 26 to 27D, as amended, apply to this project. It is the responsibility of the contractor, before bid opening, to request if necessary, any additional information on Minimum Wage Rates for those trades people who may be employed for the proposed work under this contract. Wage Rates dated August 12, 2020 shall be provided in the bid specification and are applicable to this project. Contractors shall be required to comply with all applicable Massachusetts General Law Chapter 149, and all other applicable Massachusetts General Laws.
The successful bidder will be required to furnish a Labor and Materials Payment Bond and a Performance Bond each in the amount of one hundred percent (100%) of the contract amount.

Bids are due in the Administrator’s Office no later than 2:00 p.m. on September 22, 2020 and the bid opening will be at that time. All bids must be received prior to the opening to be considered.
Prevailing wages as determined by the Department of Labor and Industries under the provisions of Massachusetts General Law c. 149, s.26 apply to this contract. OSHA 10 certification to be submitted with bid and is required for any worker performing work on Town property. The Town of Otis reserves the right to reject any or all bids when it deems to be in the best interest of the Town. A contract award will be made by the Town of Otis Board of Selectmen within 30 days after the bid opening.
