SBPHC is pleased to present their November mobile flu/vaccine
clinic schedule, including a clinic at the Otis Town Hall on
Tuesday, 11/5 from 10am – 12pm. At these clinics, we will be
providing updated Pfizer COVID vaccines, regular-dose flu
vaccine and egg-free flu for ages 6 months and older and high-dose flu (for ages 65 and older). We are also offering in-home
vaccinations to people living in member towns who have
difficulty leaving the house. To request an in-home vaccination,
please call Jill Sweet at 413 717 7209.

Registration is highly recommended for your flu or COVID
vaccine, which can be done using one of these easy methods:
• Go to https://home.color.com/vaccine/register/tritown and pick
the desired clinic;
• Contact SBPHC staff at (413) 243-5540 x109 and leave a message
with your name, phone number and reason for call and we will call you
back to register you!

Although the flu and COVID vaccinations are provided at no cost, we ask that you have
your insurance card with you at the time of registration as well as the day of the clinic.
This will reduce your wait time the day of your vaccine appointment. Our goal is to keep
our communities protected this respiratory illness season by ensuring all community
members have access to vaccines.
